TITLE: Early start cholesterol screening program forms

DESCRIPTION: These forms are used as part of a research project to collect data on cholesterol levels of third grade children in Utah. The project, which was titled Early Start for a Healthy Heart, indicated the levels of cholesterol in these children compared to other children in the nation. Information includes child's name, address, phone number, height, weight, birth date, age gender, ethnic origin, medical history, cholesterol level, and consenting signature.
