TITLE: Records transfer sheet/records disposition request and authorization

DESCRIPTION: The Records Transfer Sheet is used by the agency to transfer records to inactive storage at the State Archives. After transfer, the form is used as a reference tool to enable the agency to retrieve the record. The Records Disposition Request is sent by the State Archives staff to the agency to request their authorization to dispose of the records whose retention has expired. They include the agency name and address, a description of the records, the name and address of the records officer, the classification of the record, and the retention period.
