DESCRIPTION: These are applications, contracts, project reports, studies, certificates, agreements, memoranda, correspondence, and other records relating to the receipt, review, award, evaluation, status, and monitoring of the grants, the allocation of funds, and the project budget. Information in these files include Notification of Intent for proposed state actions, Environmental Protection Agency Assistance Agreement/Amendment, State/EPA Agreement Worksheet, Application for Federal Assistance, letter form the governor giving approval to the department to submit the grant application, and related correspondence, the applicant's name, address, employer identification number, program title and number, the title and description of the project and the area of impact, the amount of the proposed funding, the amount of money granted, a record of the money spent, including how much and on what, and the dates of all actions.
