DESCRIPTION: This form documents operators' efforts to enhance production of lower value wells. In so doing, the operators complete the first step in qualifying for a tax credit administered through the Division of Oil, Gas, and Mining, under authority of the State Tax Commission (UCA 59-5-102) (2004). The form lists operator and well information, type of work commenced and completed, operating expenses and division approved expenses, a list of contractors providing services, in-kind services, and the name, signature, and title of the operator.
