DESCRIPTION: These are agreements between the agency and the custodial parent participant receiving funds through the program. The participant submits personal banking information to the agency in order to facilitate the electronic transfer of child support funds recovered from the non-custodial parent to the specified account as the funds become available. Information includes name of program participant, date submitted, bank account number, banking institution number and authorizing signature.
