DESCRIPTION: A signed form is required for youth hunters to participate in hunts offered by the Utah Division of Wildlife Resources. The chukar and pheasant youth game hunts are examples of hunts conducted throughout Utah on state wildlife management areas. These forms contain the hunting event area name, date, youth name, parent/guardian name and signature.
