DESCRIPTION: These applications for licenses are for the sale of motor fuel as required by UCA 59-13-203, 1989. These applications include batch header cards and application for license to engage in business as a dealer in motor fuel. Information includes the dealers name and address, license number and the date issued, character of the business, business address, whether the business is a sole proprietorship, partnership or corporation, the names of the owner or partners, social security number, batch number, document count, routing of the batch, type of transaction, amount of increase or decrease of assessment and remittance and the amount of payment made.
