TITLE: Staffing reports

DESCRIPTION: These are quarterly reports submitted to the Utah State Health Bureau of Facility Review to show that the facility maintains adequate staffing at all times. These reports are required by 42 Code of Federal Regulations (CFR) 431.610(g)(1)(ii)(1989). These reports include the Report on Consultants, Report on Other than Nursing Personnel, and Report on Nursing Personnel-Supplementary Page. Information within the reports includes the name and address of the facility, report date, reporting period, name of the employee or consultant, employee's position or the consultant's area of consultation, number of hours worked by the employee or consultant, and a signed certification by the facility administrator that the information submitted is correct and that all standards are being met.
