DESCRIPTION: This is an audit report submitted annually by the center's auditor to Medicare. It is required of all Medicare providers on the "provider's accounting year" [42 Code of Federal Regulations (CFR) 413.2(b) (1989)]. This report includes the name and number of the provider, dates covered, administrator's signature and title, name and phone number of person to contact for additional information, financial data, whether report is audited or compiled, a series of questions on the provider's organization and operation, capital and related cost, interest expense, insurance, deferred compensation and pension, education activities, names of provider-based physicians, office costs of detailed accounting, amount of all Medicare funds, expenditures and revenues.
