DESCRIPTION: This records series is a mandatory evaluation required prior to admittance to the facility. Evaluations are submitted by a private psychiatrist and are used in the determination process of admittance and non-admittance to the facility.

After a person has been admitted to the facility, the file can be transferred to the client's record.

This series includes the Psychological Evaluation, out patient number, test scores, a family background or history, and any type of related correspondence.
