TITLE: Central speech and audiology files

DESCRIPTION: This is a speech and audiology file maintained on a client. It is used to make an assessment for treatment and progress from the client's first date of testing to his last date of treatment.

All evaluations, progress notes and test results are transferred to the client's records.

This series includes a Chronological Speech and Hearing Case Record, the date of testing, recommended therapy, referral for further treatment, the name of the examiner, examiner comments, copies of the medical visit forms, Problem Oriented Records Disposition which contains the problem the client is having; a diagnosis; comments; and recommendations; Evaluations, an Audiometric Evaluation which contains the testing and hearing level; results; graphs of results; recommendations; Language Scale Test, Peabody Picture Vocabulary Test, an Environmental Prelanguage Battery Test, the raw age of the client, the mental age of the client, and the Bankson Language Screening Test Score Sheets.
