TITLE: Teaching interaction motivation system monthly data sheets

DESCRIPTION: This is a record of tasks which are to be completed satisfactorily so many times per month by the resident, and the amount of points earned for each. These tasks are outlined in the resident's Individual Habilitation Plan (IHP). Satisfactory behavior moves the resident into the next higher level of independence. When he has reached a certain level, he is then considered for outplacement into another unit at the facility.

These records are not duplicated elsewhere at the facility. The building where this program is operational, is a unit for the developmentally disabled person who has committed some type of offense, but who has not been tried in criminal court. He is placed at the facility for rehabilitation, future release into a less restricted facility unit and eventual release to a community group home.

This series includes a Daily Earning Sheet, Behavioral Monitoring Sheet and the client's present behavioral level.
