DESCRIPTION: These files document services provided to individuals enrolled in the family planning program funded by federal grant funds under Section 1001 of the Public Health Act. The purpose of the grant is to provide educational, comprehensive medical, and social services necessary to aid families, particularly low income families, in determining the number and spacing of their children. These files include a physical examination form, a record of client visits, progress notes, financial form, signed consent forms, contraceptive method authorization forms, and laboratory test results.
