This is part of a federal grant from the Department of Health and Human Services. Included in these records are the name of the local health department, amount and terms of the contract, contract number; and signatures of the director of the local health department, the director of Community Health Services, the Department of Health Financial Manager, the Department of Health Director, and the Attorney General.
