DESCRIPTION: These are records which document the percent and the amount of employee and University contributions toward retirement benefits. They include the premium lists, annual reports, and deduction registers, and are needed to determine the remittance sum upon retirement. The Benefits Division of the University Personnel Office uses the records to track retirement problems. Data elements include name, social security number, earnable salary, University of Utah contributions, total premiums, total deductions, and termination date.

Microfilm duplicate: Retain in Agency Record Center for 75 years and then destroy.

Because of the high reference rate of these records, a retention that covers the possible years for claims is necessary, and then allows for the destruction of the records and the personal information contained therein. Previous decision: RDR 76-79 stipulates microfilming.
