DESCRIPTION: These files contain two separate forms completed by each sworn officer containing equipment issued. The one form is a card recording the issuance of badges, handcuffs, brief cases, and body armor. The second form is used to record the issuance of all other equipment and includes name of officer, identification number, date of birth, date of appointment, date of issue, date of return, type and number of each piece of equipment issued, and serial number.
