DESCRIPTION: These files contain records of officer's physical examinations. They are required every two years for officers under the age of forty and annually after age 40. These files contain scheduling information along with officer's name and identification number, date of birth, date of hire and date of last physical exam, These files include a log where information is accessible by date of last physical examination and includes officer's name, ID number, ID number, division, hire date, and date of birth.
