DESCRIPTION: These are claims for reimbursement submitted by outside consultants to the various bureau programs. The varying title represents an obsolete version of the form. The claim forms include the claimant's name and address, the date that the claim arose, a description of the expense being claimed, the amount of the claim, the signatures of the approving official and the claimant, and the account to which the charge was applied. The obsolete form gives a breakdown of the expenses for services, transportation, and subsistence. It also specifies if the work was for heart clinic, maternal & child health conference, TB clinic, or medicare.
