It includes application for emergency medical services certification, copies of written and practical tests, notification of test results letters, application for emergency medical services recertification, notification of incomplete certification, verification of certification status, and training records. Information includes appearance, civil/criminal court involvement, current and past addresses, date of birth, driver's license number, educational level, employer, employment history, job position information (grade/step, etc.), name, occupation, occupational preferences, occupational licenses, physical characteristics, sex, signature, social security number, telephone number.

The paper file is needed in the office, according to the staff, as long as the individual is certified. Information is regularly added to the file as long as it is active.
