DESCRIPTION: These are contracts with local agencies to carry out the EPSDT program. This program is designed to provide early and periodic screening and diagnosis of eligible medicaid recipients under age 21 to ascertain physical and mental defects and to provide treatment to correct these conditions. These records include a copy of the contract and related correspondence, the name of the contractor; the effective date of the contract; the termination date of the contract; the amount of the contract; the terms of the contract; and the signatures of the contractor, the division director, the department director, the attorney general, and the director of the Department of Finance.
