DESCRIPTION: This is a review by the bureau to verify that patients admitted to medical institutions need to be there and are receiving proper care. This information is required by 42 CFR 456_(1990), Sections C, E, and F, and documents individuals on the Medicaid program. The information is a safeguard against unnecessary utilization of care, services, and health care facilities. Information includes preadmission/continued stay inpatient care transmittal forms, patient care transmittal, continued stay review, and contact notes. Includes an evaluation of the recipient's need for assistance, a plan of care, medical review, certification by a physician that inpatient care is needed, provider, provider identification number, name of the attending physician, and any changes to the patient's condition or plan of care.

The information documents medical information and serves as a medical history for individuals on the Medicaid program. Therefore the information has the same retention as a typical medical record.¶¶Previous decision was three years. The agency believes a longer retention is necessary to ensure proper documentation concerning the patients medical history.
