DESCRIPTION: These records are appeals, made by Medicaid recipients, to decisions of the Medical Review Bureau stating that continued or proposed medical treatment of said recipients is unnecessary.

Included in this record are a request for an informal hearing; notice of informal hearing; notice of denial and hearing rights; and notice of hearing results. Information within the forementioned notices include the name, telephone number, address, and social security number of the client and of the client's representative; the symptom or condition for which payment of services was denied, and the reason for the denial; medical information; conditions involved; the decision of the hearing officer; signatures of hearing officer(s) and of the approving official(s); and the dates of hearing(s), decision(s) and notification(s).
