DESCRIPTION: Record of all injuries requiring the loss of a half day of school or medical attention to students in the public school system for injuries occurring during school hours. Includes the nature of the injury, the contributing factors and any actions taken. Information is also used to document the student's injury history while attending public schools and for statistical purposes by the department. The file includes the child's name, sex, birth date, grade, parent's name, accident date and time, school name and number, type of injury, area affected, contributing factors, action taken at the school and who took the action, witnesses name and address, surface and location where the accident occurred, activity during which the accident occurred, period when the accident occurred, and parents and school follow-up action.
