DESCRIPTION: This is part of a federally funded project to research the cause of Sudden Infant Death Syndrome. This program is funded under 42 USCA 289d and 300c. These records investigate the family background and response to SIDS. The file includes Sudden Infant Death Syndrome Case Information, Nurse Referral Form, and notice from Medical Examiner's Office. Information includes the infant's name and date of birth; the date and time found dead; the place of death; the infant's medical history; the maternal and gestation history; record of other SIDS cases in the family; socioeconomic and environmental data; the parents' names, address, dates of birth, religion, educational level, and occupation; the parents' and siblings reactions; the dates of visits; and the name of the nurse or counselor.

Administrative Historical

The only federally mandated retention for these records is that of 45 CFR Part 74, which addresses grant related records in general. This requires a three year retention. However, due to the high research and sociological information in these files, a permanent retention is warranted. Due to the relatively small quantity of records and budget problems, retention of the paper copy is recommended. However, this should be reviewed at a later date.
