DESCRIPTION: This is a reference card file of all SIDS deaths in the state. This is used both for quick reference and to provide documentation of repeated cases of SIDS in families. This file includes the child's name, age, race, and dates of death and birth; the parents' names date of birth, age, education level, occupation, and religion; the name and address of a friend or relative and of a baby sitter; the diagnosis; the date referred to the bureau; the name of the referring individual; the date the case closed and how it closed; and a record of the services given and the family's adjustment.
