DESCRIPTION: This is a file of specialists to whom the bureau sends children for treatment. In order for a physician to be a consultant for the bureau, they must apply and be accepted. The file includes the doctor's or dentist's name, address, date and place of birth, education, training, licensure date and number, whether the license was revoked or the physician convicted of a criminal offense, present hospital staff membership, membership in groups, speciality, and signature.
