DESCRIPTION: This is a record of billings made to the federal government and to private insurance companies for patient services provided by the division's programs. The file is used to ensure that all potential other sources of payment have been exhausted before the state pays for the treatments. This record series includes the patient's name, address, sex, date of birth, chart number, employer, insurance company, telephone number, and medicaid number; the health care provider's name, address, social security number, employer identification number, and telephone number; the services being billed for; and the amount of the charges.
