DESCRIPTION: This is a record of a patient who has been referred to the Division of Rehabilitative Services for vocational rehabilitative counseling as outlined in an agreement between the Utah State Hospital and the Division of Rehabilitative Services. The patient's needs are assessed in regards to work skills, background education, interests and motivations, physical abilities, experience, and work habits. Through this service, the patient can slowly adjust to the demands, expectations, and stress of working in the community.

This series includes a Client Statistical Record, Initial Interview Form, Notice of Acceptance, Request and Authorization for Release of Confidential Information, Individual Written Rehabilitative Program, Job Application, finances, Determination of Need for Financial Supplementation, Disability Consultation, Report of Electroencephalography, Evaluation of Medical Study, Radiology Report, Neuropsychological Assessment, Closure Statement, Career Assessment Inventory Test Sheet, doctor's name and signature.

The State Hospital is currently developing their own rehabilitative program. These records are currently in the possession of the Division of Rehabilitative Services. The records or copies of the records that are on State Hospital Patients should be given to the State Hospital for an orderly transition or the program, for the continuation of quality patient care, and to meet the Joint Commission of Accredited Hospitals requirements.
