It includes Referral and Application for Service, Problem-Oriented Perinatal Risk Assessment, Psycho/Social Evaluation, Nutritional History, Health History, and Physical Examination results. Information includes age, alcohol or drug addiction, appearance, assets and debts, condition of living quarters, current and past addresses, date of birth, educational level, employer, ethnic group, expenditures, family history, food purchase and consumption, home ownership, income, marital status, medical information, motor vehicle ownership, name, name of kin, national origin, number of children, occupation, personality inventory, physical characteristics, physical disabilities, psychiatric information, public housing occupancy, race, salary, sex, social security number, and telephone number.
