DESCRIPTION: These are computer generated mailings to the family of a child identified as a high risk for hearing loss. It notifies the parents that their child may have hearing problems and invites them to have their child's hearing tested. The parent returns the card to request a hearing test or to indicate no interest in the test. The information includes the parents' name and address, and whether the parent wants the child's hearing tested or not, and, if so, where they want the testing to be done. If the patient is not tested, a copy of the card indicating that testing was offered and refused should be kept in case the parent later attempts to bring legal action against the state.
