DESCRIPTION: This is a annual report which documents the admissions, discharges and deaths of the client on a yearly basis. This record is used to compile death reports and to compare billings.

This series includes the facility where the patient is housed, code number, number of clients, the number of beds allotted and available, the number of home and temporary visits made by the client, file number, admission type, where they were admitted from, where they were discharge, the client's name, the number of admissions; discharges; deaths and transfers.
