DESCRIPTION: These forms are required by the airport to be signed by persons needing assistance in cases where their automobile will not start. This service is provided free of charge to airport patrons and this form releases the airport from liability in the case of any problems that may arise. These forms include statement of indemnity agreement; date and time called; signature of person being assisted; make, model, year and license number of automobile; name of persom assisting; and amount of time taken.
