DESCRIPTION: These forms are signed by the president or vice president of a partnership or corporation acknowledging that they will be responsible for payment of water bills in case of the dissolution of the corporation or partnership. They are used to determine accountability for water billing. These forms include company name, signature and title of responsible party, mailing address, driver's license number, social security number, date of birth, date signed, service address and account number.
